  • I’d try to trace the upper border of that main artery shadow as best I could.
  • Then, I needed the level of the diaphragm. Usually, I looked for the highest point of the right hemidiaphragm because it’s often clearer.
  • Then came the measuring part. Just drawing a vertical line, basically, from that aortic knob level down to the diaphragm level.

I did this on a few different images. Sometimes I used a simple image viewer with a measuring tool, other times I was literally holding a ruler up to a screen printout – pretty basic stuff, just trying to get a feel for it myself.

What I Found Out

Honestly? It wasn’t as cut-and-dried as I thought. Getting consistent measurements was tough. The angle of the X-ray seemed to change things a bit. Sometimes defining that exact ‘top point’ felt like guesswork. One image, I’d measure X, look at another apparently similar one, measure Y, and they weren’t quite the same.

It really drove home that looking at these things requires a trained eye and probably better tools than what I was using. It’s not just about knowing the points A and B, but really understanding the image, the anatomy, and potential variations. You need context.
